Modification Request Template Format
1. Introduction
To assist Approved ISPs and JANET Customers in completing the Template for Requesting Modification to a Domain Name Under Ac.uk/Gov.uk when applying for modification to existing domain name entries under ac.uk or gov.uk, please read the following.
It is the responsibility of the new Approved ISP/JANET site to submit the modification request template. The minimum information required for completion of each modification request template is:
- Domain name;
- Domain owner/customer requesting the modification;
- Administrative contact name and details;
- Technical contact name and details;
- Registrant contact name and details;
- Names and corresponding IP addresses of at least two working nameservers;
- JANET(UK) Approved ISP Membership Account Number, if applicable;
- Previous ISP name, if known.
If the name owner is changing ISP for its domain name, then a confirmatory fax is also required, as per section 3 of the Procedure.
2. Attributes
The general format for a modification request is:

- The attribute name includes the terminating colon -:
Values such as addresses (address:), which may continue across multiple lines, should have the attribute name at the start of each line.
The attributes are:

| representing: | name of project, initiative or organisation for which the name is being used |
| domain owner: | name of organisation that owns the domain |
| admin-c: | Administrative contact = a person |
| address: | full postal address of the admin contact |
| phone: | telephone number of the admin contact |
| fax-no: | fax number of the admin contact |
| e-mail: | electronic mail address of the admin contact |
| tech-c: | Technical contact = a person |
| address: | full postal address of the technical contact |
| phone: | telephone number of the technical contact |
| fax-no: | fax number of the technical contact |
| e-mail: | electronic mail address of the technical contact |
| reg-c: | Registrant contact = a person |
| address: | full postal address of the registrant contact |
| phone: | telephone number of the registrant contact |
| fax-no: | fax number of the registrant contact |
| e-mail: | electronic mail address of the registrant contact |
| nserver name & ip address: | names and corresponding IP addresses of nameserver serving the domain name |

| Approved ISP Mem. No: | Membership number obtained from joining Approved ISP Membership Account, if applicable. |
| Previous ISP: | The previous ISP, if known. |
